Aquacare and Fitness Forum Physical Therapy are proud to partner with Limber, an app that allows our patients to continue their exercises at home. With simple explanations and videos, Limber keeps our patients moving even when they are at home.
What is RTM?
RTM, which stands for “Remote Therapeutic Monitoring,” is a way for your physical therapist to keep track of your progress and how you’re doing with your exercises even when you’re not in the clinic, using technology like a smartphone app or a wearable device to report on your pain levels, how well you’re doing exercises, and any other important details about your recovery, allowing them to adjust your treatment plan based on your real-time progress between in-person sessions.
At Aquacare and Fitness Forum Physical Therapy, we use the Limber app. This app is a great way to view your prescribed exercises, watch videos, and track your personal wellness goals.

Meet Your Care Navigators: Sally & Cait
As part of our team, Sally and Cait will personally check in with you throughout your therapy journey. They’ll call to see how you’re doing, answer questions about your home exercises, and ensure you’re staying on track toward your goals. If you need any adjustments, they’ll work directly with your therapist to update your plan.
