If you are unable to attend your appointment, please call us as soon as possible. Early notice allows another patient the opportunity to receive the care they need.

Please note that missed first appointments or cancellations without adequate notice may result in a $25 missed appointment fee.

FAQs

Please bring:

  • Your photo ID
  • Your insurance card
  • Any referral or prescription from your physician (if required by your insurance)
  • A list of your current medications (if available)

Your licensed physical therapist will:

  • Review your medical history and current symptoms.
  • Perform a thorough evaluation.
  • Discuss your goals and answer your questions.
  • Develop a personalized treatment plan designed specifically for your needs.

Please tell your therapist what you hope to accomplish. Whether your goal is returning to work, playing with your grandchildren, getting back on the golf course, or simply living with less pain, your treatment plan will be designed around your goals.

Initial Evaluation: Approximately 45–60 minutes

Follow-up Visits: Typically 40–60 +minutes, depending on your treatment plan.

Please arrive 15 minutes early if you have any remaining paperwork.

Please wear comfortable clothing that allows you to move easily and supportive athletic shoes.

If your physician has referred you for aquatic therapy, your first visit will be an evaluation, so you will not be getting into the pool during that appointment.
