Form 1095-C Availability Notice

In accordance with Internal Revenue Service (IRS) regulations, copies of Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, are available upon request.

Form 1095-C provides information about health coverage offered to you during the prior calendar year.  This form is only provided to employees who worked full-time at any point during the prior year.

This form is not required to be submitted with your federal tax return; however, you should retain it for your records.
